GUIDELINES TO ALL UAT APPLICANTS
1. The cellphone number that you will use upon signing in app.mis.asu.edu.ph/application must be unique and must not be duplicated. To reiterate, ONE CELLPHONE NUMBER FOR ONE APPLICANT. Strictly no sharing of Cell Phone Number with other applicants.
2. The email address that you should provide upon registration must be unique and must not be duplicated. To reiterate, ONE EMAIL ADDRESS FOR ONE APPLICANT. Strictly no sharing of email address with other applicants.
3. Make sure that your EMAIL and CELLPHONE NUMBER ARE ACTIVE for some important reminders and announcements will be sent there.
4. Please provide CORRECT AND COMPLETE INFORMATION. Fields marked with an asterisk (*) are required.
5. INCOMPLETE APPLICATION. NO UAT EXAMINATION.
Failure to follow these important requirements will disallow you to LOGIN the system and take the UAT examination.

HOW TO APPLY FOR THE UNIVERSITY ADMISSION TEST (UAT)?
1. Go to app.mis.asu.edu.ph/application to access the UAT Application System and click SIGN UP HERE.
2. To sign up, please provide the required fields then click SIGN UP.
NOTE:
- Provide CORRECT and COMPLETE INFORMATION.
- The CELLPHONE NUMBER that you will use must be unique and must not be duplicated. Strictly no sharing of cellphone number with other applicants.
- The EMAIL ADDRESS that you should provide must be unique and must not be duplicated. Strictly no sharing of email address with other applicants.
- Make sure that your email address and cellphone number are ACTIVE.
- Take note of your USERNAME and PASSWORD to avoid any inconvenience in the future. Make sure that your password and confirmed password MATCH.
3. You are now redirected to the DATA SUBJECT CONSENT FORM. Read and understand the consent form then click AGREE to proceed.

4. Upon successful sign up, you are redirected to the REGISTRATION FORM. You must now provide all the necessary information for your UAT Application.
NOTE: Please provide CORRECT and COMPLETE INFORMATION. Fields marked with an asterisk (*) are required. The system will not accept your application in case of failure to provide the field requirements.
5. After filling out all the required fields, please provide the necessary ATTACHMENTS (2X2 PICTURE, REPORT CARD and ELECTRONIC SIGNATURE) then click SAVE.
NOTE: File format must be strictly followed. Read the instructions carefully for each required attachment.
6. SAVE YOUR INFORMATION by clicking OK.
7. Next, click the UPDATE PROGRAM/S and choose the First and Second Programs you intend to enroll in from the dropdown menu.
8. You have successfully applied for the University Admission Test (UAT). You may now view your profile. Please take note of your APPLICANT NUMBER before logging out of the system.
Please visit the ASU-MIS Portal (mis.asu.edu.ph) as frequently as possible for advisories and announcements. Your UAT schedule and website link will also be posted at the portal.
